Subject: Hesitation when in cruise

Location: where ever hubby takes me

My Dodge Dually V10 has a bad hesitation when I have Cruise on.  It will actually jump from like 1900 to 2500 rpms and then drop back down.  Anyone have any idea what it could be,  it did it before i blew two pistons and it did it right after the new rebuilt engine went in.
